Birth declaration [🇬🇧]
The point of the birth declaration required by law is to register the newborn baby in the municipal registry of civil status, thus allowing the baby to acquire all the rights provided for by law.
How can you present it?
- Within 3 days of the birth directly to the birth centre located in the hospital. In this case Ausl di Piacenza sends the official birth declaration to the municipality where both parents (or just the mother) live, that means the municipality where the baby was actually born, as provided for by the law.
Here, in the hospital in Piacenza, it can be submitted to the birth declaration office. - Within 10 days of the birth in the municipality where both parents (or just the mother) live, if otherwise agreed, or in the municipality where the baby was actually born.
The civil registry office of the municipality of Piacenza accepts birth declarations only upon reservation. Parents are advised to promptly provide the birth declaration, as the delay or failure to register the newborn with the civil status of the municipality entails - after 10 days - the assignment to the public prosecutor by the institution.
What is needed?
To declare the birth of a child born in the hospital, parents must own the birth certificate issued by the obstetric staff present at the birth. It is then necessary to show a valid identity document such as an identity card, a driving licence, a passport or a residence permit with a photo attached.
The declaration must be made in the presence of both parents in the following cases:
- unmarried parents
- married parents who intend to give the newborn a double surname

Registration for the Sistema Sanitario Nazionale (SSN)
For newborns resident or assisted by Ausl di Piacenza, the office that accepts the birth declaration will forward it to the Revenue Agency to get the newborn's tax ID code. When parents receive the newborn's tax ID code, they will be able to choose a pediatrician when registering to the Regional health service.
With the attribution of the tax ID code, the newborn's first health card will be also issued and sent by the Revenue Agency to the parents' address.
For newborns residing or cared for outside the province of Piacenza - so that the hospitalization is paid for by the SSN -, after registering to the chosen Regional healthcare service, the parents should send via fax (0523.303191) to the acceptance office of the hospital in Piacenza a photocopy of the registration certificate and the tax ID code of the newborn baby.
Hospitalization will thus be paid for by the SSN.

For parents registered with the Regional health service it is possible to activate the Electronic health record (Fse) for their children. Fse is the collection of health documentation available on the internet in a protected and confidential area.
By activating it, you can check online the results of visits and tests, the reports of many public structures that are part of the Regional health service, your pediatrician's services, the status of your vaccinations, and many other useful information.
